The admissions process begins with a collaborative exchange of information aimed at determining whether Stonebrook, Inc. can support the needs of a potential resident.  During the initial exchange, Stonebrook, Inc begins to understand the scope of the individual’s challenges and personal goals. Consideration may then be given to the combination of services and resources necessary to best support a candidate.

The process continues with a Pre-Admission Screening (PAS), which is Stonebrook's opportunity to interview and evaluate the candidate.  A program tour is offered to the individual, their family, and/or support service provider so that everyone involved in the decision-making process has an opportunity to consider Stonebrook’s residences first hand. If you are unable to plan a personal visit, please be sure to take a virtual tour on our website (www.stonebrookdisability.com). We will take into account the feelings, personalities, and needs of our current residents when deciding upon specific house placement, so as to ensure a smooth transition. It is often necessary for the potential applicant to spend a few days at one of the homes for a trial visit. Individuals who are non-verbal, deaf, visually impaired, or semi-ambulatory will be considered on an individual basis.  Service support is explored and confirmed during the referral process.  Following the PAS process, a list of proposed services and available service support is presented for consideration. 

We ask that you please provide the most recent copy of psychological evaluation, social history, and medical history.

Individual must be diagnosed with intellectual and or developmental disabilities and be over the age of eighteen. Many of our residents are dually diagnosed (mentally ill and developmentally disabled or have significant behavior issues).

It is important that prospective residents meet the eligibility requirements for Medicaid Waiver from the Behavioral Health Center or the West Virginia State Office of Developmental Disabilities. Our Service Coordinators will be glad to answer any questions you may have about the Waiver program and assist you with the application process.
